A new analysis maps 20 years of progress and setbacks across Italy’s regions.
Italians live longer than they did two decades ago, but the benefits aren’t shared equally—and anxiety and depression rose sharply during the pandemic years, according to a national study in The Lancet Public Health. The research used Global Burden of Disease 2021 data to track life expectancy and illness across all 21 regions from 2000–2021.
By the numbers
- Life expectancy climbed from 79.6 years (2000) to 83.4 (2019), dipped to 82.2 (2020) during COVID‑19, and partly rebounded to 82.7 (2021). Healthy life expectancy in 2021 was 70.9 years.
- Regional divide: Northern regions generally live longer and lose fewer years to early death than the South and Islands—though the North bears more day‑to‑day illness because its population is older. The North was hit harder in 2020 but rebounded in 2021; outcomes worsened in the South and Islands.
- Mental health: From 2019 to 2021, anxiety disorders rose 19.8% and depressive disorders 17.3% and have not returned to pre‑pandemic levels.
- Living with illness: The top causes of disability are low back pain, falls, and headache disorders. Disability linked to Alzheimer’s disease and diabetes has grown steadily over time.
Why it matters
Italy’s population is aging, and women—who live about 4.5 years longer than men—carry a heavier burden of non‑fatal, chronic conditions. The authors argue that closing the North–South health gap will require stronger, more equitable services, alongside prevention that targets behavioral and metabolic risks and supports healthy aging.
Caveats
Some outpatient and mental‑health data are incomplete, and the 2019–2021 window may understate COVID‑19’s full impact, so estimates for specific conditions carry uncertainty.
